A receipt showing payment for the flu shot must be dated between 11/7/10 – 11/30/10. No purchase of LYSOL® products is required. Mail the original, dated receipt along with the completed rebate form. Your request must be postmarked by 12/30/10.
There is a limit of one offer submission per name, address or household. Allow 4–6 weeks for delivery.
Had only I received my flu shot a little bit earlier...a friend said she heard it can take 2 weeks to be effective...I had just gotten my flu shot on Friday and then got sick on Monday. :c(
